**NEW BENEFIT OFFERING – Effective July 1, 2008**

Child Care Voucher Program

Loyola College announces its Child Care Voucher Program available to benefits eligible employees (full time, four-fifths, core or half time), who have children in licensed day care and/or before and after school programs.  This program will be effective July 1, 2008.

For the 2008-2009 benefits year, individuals eligible for this program may receive 10% of the average cost of monthly day care expenses (based on a survey of the cost of care in the surrounding area).     

Once per year, employees will have to provide a copy of their tax Form 1040 showing eligible dependents and a copy of their day care provider’s license.  If employees change day care providers, a copy of the new license will be required.  Monthly Cost Verification Forms for reimbursement must be submitted monthly and checks will be issued monthly.

Employees using this program must reduce their dependent care flexible spending accounts by the amounts received from this voucher program. The Human Resources website contains information on the current monthly voucher amounts based on a dependent’s age and/or type of care.

For reimbursement, please complete the Child Care Voucher Program Application and the Child Care Voucher Monthly Cost Verification Form and submit them to the Benefits Unit.
