Community Award

Purpose: To recognize a staff employee displaying extraordinary initiative and dedication in their day-to-day performance for: enhancing the College or greater community environment in keeping with Loyola's mission, vision and values; developing and or implementing a practical and cost or time-saving strategy; or advancing safety awareness.

Procedure: Please complete this form and submit to the Human Resources Department at 5000 York Road. The Human Resources Department will forward the nomination to the appropriate supervisor for additional comments and place a copy of the nomination in the employee's personnel file. The completed nominations will be considered by the Recognition Committee for the Community Award. Nomination deadlines are April 1 to be awarded at the Spring Staff Luncheon and October 1 to be awarded at the Fall Staff Luncheon.
