
Part-Time Affiliate Faculty Hiring Policy for New Affiliate Faculty HRIS receives and reviews contract, employment recommendation, etc. sent from department advising of new affiliate faculty. Documentation must include the following: GL, Department, Employment Start Date (Fall/Summer/Spring), Confirmation that it is an affiliate. HRIS receives and reviews the following documents sent from faculty or department: - Background Screen Request form (Employment Unit will request Background Screen)
- ER or contract containing: Rank, GL, Department, Tenure status, Salary, Employment Start Date (Fall/Summer/Spring)
- MW-507 (Maryland Tax Deductions)
- W-4 (Federal Tax Deductions)
- I-9 (Verification of Work Eligibility)
- Employment Data Sheet
- Confidentiality Agreement
- Direct Deposit Form (optional)
- HRIS sets up faculty in Colleague (see below for details)
Returning Faculty If a faculty has not been working at Loyola for more than a year and a day, a new I-9 Employment Verification form must be completed. It is also recommended that new tax forms (W-4 and MW507) be completed. Find HR Forms here: http://www.loyola.edu/HR/Forms/
