Packets are provided for each course taught. The packets are labeled with the faculty name, course name, course code, enrollment figure and location. There are return labels on each envelope directing the students to return the completed forms to the administrative assistant of each department.
Each packet contains pre-printed forms. No course/faculty information has to be bubbled in. (Faculty ID Codes are no longer used).
Each packet should have the exact amount (if not more) of forms needed according to the enrollment figures. If more forms are needed, please call the Kristen Fisher at ext. 2261. Do not photocopy evaluation forms. Photocopied forms will not be able to be scanned and analyzed.
You don’t have to provide pencils. Students should use blue or black pen only to fill out the forms.
Administrative Assistants are responsible for getting the packets to each faculty member. Please notify the instructor if you plan to send their packet to their Timonium or Columbia office.
If a faculty member misplaces their forms, email and call Kristen Fisher (kfisher1@loyola.edu, ext. 2261). A replacement packet will be made and delivered to the faculty member. We have procedures in place to handle situations should they arise at the Timonium and Columbia campuses.
The evaluation packets will be returned to you. Be sure the envelope is sealed and that the student has signed it. Sign your name on the packet as well before you or your designee return it – unopened and hand-delivered – to the Office of Academic Affairs.
Return the forms the same day the student drops them off in your office. Do not wait until all the student evaluation packets in your department are in before returning them to Academic Affairs. \
Faculty will receive a summary report of their data electronically after grades have been submitted. The completed evaluation results will be returned to the Administrative Assistant of each department. You are responsible for distributing them to the faculty.
