Federal Work-Study Forms All of the employment forms you will need to complete as part of the Federal Work-Study process are available to download from the Financial Aid web site. It is important to remember that you may not begin working until all of the required work-study forms have been completed and submitted to Human Resources for processing. PRIOR TO FINDING A JOB: Obtaining a work-study position is the responsibility of the student through a standard job search process. Once you have reviewed the listing of all campus positions you should identify several in which you are interested. Your next step is to contact those offices and follow their application procedures. Many supervisors will ask you to submit a completed Loyola University Student Employment Application. You may download and print as many copies of this employment application as necessary. I-9 REQUIRED OF ALL NEW WORK-STUDY STUDENTS: All new work-study students (those who have never worked at Loyola University) must complete the Federal I-9 form that confirms your identity and your eligibility to work. Print out and complete the I-9 prior to arriving for the fall semester. You will need to submit the completed I-9 along with the original documents required to your supervisor once you have been hired. Your supervisor will also need to sign the I-9. REQUIRED PAYROLL FORMS: Any student who did not work on campus during the previous spring semester must complete all of the following documents before you will be paid: Maryland Withholding Certificate (MW 507) You must complete this form even if you are not a Maryland resident. W-4 Direct Deposit Form Attach a voided check from the account to which you would like your earnings to be directly deposited.
Loyola University Maryland Confidentiality Agreement **Please note: If you worked at Loyola College during the previous spring semester, you do not have to complete any of the required payroll forms listed above unless your banking account or tax withholding information has changed. STUDENT EMPLOYMENT AGREEMENT: Once you have been hired for a work-study position, have your supervisor sign the Student Employment Agreement Form. A copy should be sent to The Office of Financial Aid and another copy should be submitted to Human Resources along with all of the other required work-study forms indentified above. |
