HealthyBlue HSA

HealthyBlue HSA (www.carefirst.com)

HealthyBlue HSA is a Qualified High Deductible Health Plan (QHDHP) that allows for participation in a Health Savings Account (HSA). The plan utilizes the BlueChoice HMO network of participating in-network providers while allowing the freedom of both in and out-of-network coverage. All services excluding preventive care are subject to the deductible which must be met before plan benefits will begin.

Accessing Care

The Member’s Primary Care Physician "PCP" will coordinate routine care and provide treatment for a variety of medical conditions. If more specialized care is required, the PCP will refer the Member to a Specialist or other Contracted Health Care Provider.

Contraceptive and sterilization services and procedures are not covered under any of the Loyola sponsored medical plans. These services are available directly from CareFirst. Access the benefit by providing your insurance card as you normally do to an in-network pharmacy (services and procedures will not be covered out-of-network). CareFirst will in turn arrange payment directly to the pharmacy. Contact CareFirst Member Services at 1-800-628-8549 for additional information.

Mental Health/Substance Abuse

The Plan provides inpatient, intermediate and outpatient care. Mental health and substance use disorder services must be coordinated through the mental health and substance abuse administrator rather than through the PCP.

Prescription Drug Plan

Participants in the plan automatically participate in the prescription drug plan administered by CVS CareMark. You can choose between retail and mail order options. Select generic prescriptions are available to you at no cost even if you haven't met your deductible. View the most up-to-date list.

If you are taking maintenance prescription medication, we encourage you to investigate the mail order option. Members can sign up for mail order today by calling CareFirst Pharmacy Services at 1-800-241-3371, through My Account, or by completing the Mail Service Pharmacy Order Form.

Healthy Reward

If you are enrolled in the HealthyBlue HSA medical plan you are eligible to earn a Healthy Reward. To earn your Healthy Reward, follow these steps within 120 days of the plan’s effective date.

  • Step 1: Select a Primary Care Physician (PCP) when you enroll in the plan.
  • Step 2: Complete the Health Assessment online (members 18+). Login to MyAccount at www.carefirst.com/myaccount. Employees without internet can call 1-866-454-5375 to request a paper copy of the Health Assessment.
  • Step 3: Work with your PCP to complete the Health and Wellness Evaluation Form. The completed form can be faxed to CareFirst or you can enter the results by logging in to MyAccount and clicking My HealthyBlue Rewards. From the qualifications tab, click Begin Evaluation and enter your results.  

Once you complete the three steps, CareFirst will reduce your deductible by $200 for the current plan year beginning July 1st. In a family, each adult can earn a $200 reward for completing the three steps, while children ages 2-17 can earn $25. However, the maximum reward for a family is limited to $400 for the plan year beginning July 1st.
