New employees must complete the following forms, available from our "Forms" page:

  • I-9 Employment Eligibility (New hires must complete section 1 of the Form I-9. The employer completes section 2. Loyola's employer code is 14129. Employment verification must be completed within the first three days of employment. Please bring the required original documents to human resources for verification.)
  • W-4 Federal Tax Withholding certificate
  • MW 507 State Tax
  • Confidentiality Agreement
  • Direct Deposit Agreement
  • Personal Data Form

Eligible new employees may participate in benefits the first month coinciding with or following their date of hire or eligibility date, depending on the benefit. New employees must complete the following within 30 days of their start date:

  • Online Benefits Enrollment
  • Medical Default Notice (complete and return to the benefits unit)
