Checklist

Below is a list of the information necessary to process your transcript request quickly and efficiently.

  1. Name (include middle initial and any former or maiden names, if applicable)
  2. Address (include zip code)
  3. Social Security or Student ID number
  4. Date of Birth
  5. Daytime phone number (direct extension, if applicable)
  6. Cell phone number
  7. Approximate dates of attendance
  8. Degree received (if applicable)
  9. Official or unofficial transcript needed
  10. Number of copies
  11. Name of third party (school, company, or organization)
  12. Mailing address of third party
  13. Deadlines (if applicable)

Please provide the following information for official transcripts that need to be included in an admissions packet or hand-delivered to a university, company, etc.

  • Name of the third party that you will be forwarding the transcript to, as their name will appear on the sealed envelope in addition to your name.

Please note:

  • This method can only be used to request paper transcripts.

  • Upon receipt of an official transcript, do not open the envelope as this will render the transcript unofficial, and the recipient may require you to provide another copy.

  • If you wish to see your transcript, be sure to request an unofficial copy for your review.

  • For unofficial transcripts, please include the complete mailing address.
