We are pleased to inform you that the student health insurance plan for the 2015-2016 academic year has changed insurance carriers from Aetna Life Insurance Company to United Healthcare Insurance Company.
Here is a brief overview
of the Loyola-sponsored Student Health Insurance Plan for the academic year 2015/2016. Information can be found on www.firststudent.com
Deadline to enroll is August 31st.
You are eligible to voluntarily enroll in the Loyola-sponsored Student Health Insurance Plan if you are a full-time or part-time graduate student in a degree seeking program taking at least 6 credits. Graduate students taking less than 6 credit hours are eligible if they are in a degree-seeking program and taking coursework to complete their degree with the University for the current policy year.
To enroll in the plan, please complete the application (www.loyola.edu/graduate-health-insurance). Once this application is submitted, a representative of RCM&D will follow up with you regarding registration instructions.
The rates for the 2015-16 Loyola-sponsored Student Health Insurance Health Plan are:
Annual – $1,581 (8/15/15 – 8/14/16)
Fall only – $600 (8/15/15 – 12/31/15)
Spring only - $981 (1/1/16 – 8/14/16)
Summer only - $458 (5/1/16 – 8/14/16)
