Complete the college health forms. It is recommended that you make an appointment with your health care provider when you receive the form so that you can have it completed and receive any necessary vaccinations by July 20, 2009. Be sure to complete both sides and do not send attachments. The Health Center Web site has links to further information on these forms. Complete math and language placement tests Completion of mandatory math and language tests is a necessary step in your academic preparations for summer orientation, as this will ensure that we can advise you properly in your course selection. Information will be sent to you in the academic materials packet sent out on May 9th. Please complete the required tests by June 12. Intercollegiate athletics. If you are planning to play intercollegiate athletics, or even think you may, you must also complete some additional health forms and information.
