Required Next Steps

Complete the University health forms

It is recommended that you make an appointment with your health care provider when you receive the form so that you can have it completed and receive any necessary vaccinations by July 25, 2011. Be sure to complete both sides and do not send attachments. The Health Center website has links to further information on these forms. 

Complete math and language placement tests

Completion of mandatory math and language tests is a necessary step in your academic preparations for summer orientation, as this will ensure that we can advise you properly in your course selection. Information will be sent to you in the academic materials packet sent out on May 2, 2011. Please complete the required tests by June 15, 2011.

Intercollegiate athletics

If you are planning to play intercollegiate athletics, or even think you may, you must also complete some additional health forms and information. You can do so by downloading the Freshman & Transfer Student-Athlete Health Information Packet on the athletics website.

Students with documented disabilities

All students with documented disabilities should register with Disability Support Services. This office arranges services and accommodations for students with disabilities in order to provide academic, physical, and electronic access to all University programs. Students will be required to provide current and comprehensive documentation of disabilities. Visit the Disability Support Services website for more information.
