Any items left blank or filled out incorrectly on the Federal Direct Stafford Student Loan Graduate Information Sheet will delay the processing and awarding of your Federal Direct Unsubsidized Stafford Loan.

:  Please provide your first and last name.

Loyola ID or Social Security Number: If you don’t know your Loyola ID, please provide your social security number or last four digits of your social security number.

Home Telephone Number: Please provide your home telephone number if available.

Cell or Work Telephone Number: If you cannot receive calls at work, please provide your cell telephone number.  

Email Address: If you do not have a Loyola University Maryland email account at the time you fill out this form, please provide an email account that you check regularly.  Once you receive a Loyola email account, all correspondence initiated by the office of financial aid will go to your Loyola email.  It is important that you check your Loyola email account regularly. 

What semester and year will you or did you start your current graduate academic program? If you are a new graduate student, please provide the semester and academic year you will start your program.  

Total number of credits earned toward degree to date: If you don’t know give a best estimate.  If you are a new student, write in 0 (zero).  

Full-Time Program:  Full-time programs are academic programs that charge a flat tuition rate for the entire academic year.  These programs do not have a per credit tuition rate.  If you don’t see your program listed under the drop down menu for the Full-Time Program, please go to the Per Credit Program.  

Per-Credit Program:  Per-Credit programs are academic programs that charge a per credit tuition rate. Most Loyola University academic programs are charged per credit.  

Planned credits, Summer I: You must be a minimum of 3 credits to be eligible for Federal Direct Unsubsidized Stafford Loans.  If you do not want loans for a particular semester, please leave this blank or write in zero.

Planned credits, Summer II: You must be a minimum of 3 credits to be eligible for Federal Direct Unsubsidized Stafford Loans.  If you do not want loans for a particular semester, please leave this blank or write in zero.

Planned credits, Fall Term: You must be a minimum of 6 credits to be eligible for Federal Direct Unsubsidized Stafford Loans.  If you do not want loans for a particular semester, please leave this blank or write in zero.

Planned credits, Spring Term: You must be a minimum of 6 credits to be eligible for Federal Direct Unsubsidized Stafford Loans.  If you do not want loans for a particular semester, please leave this blank or write in zero.

Graduation Date: Please provide month and year of your anticipated or known completion date.

Have you ever borrowed from the Federal Stafford Loan Programs? Please let us know whether you have borrowed through the Federal Stafford Loan program in the past.  It doesn’t matter how long ago or how recent.

Will you receive any other financial assistance? Other financial assistance includes fellowships, graduate assistantships, state scholarships, tuition reimbursement from your employer, etc.
