Faculty Forms List

The following is a list of forms for faculty to request services from the LLC. Please select and complete the appropriate electronic form and you will be contacted by an LLC staff member shortly thereafter to confirm your request or reservation. Remember, you can also send your request via e-mail to LoyolaLLC@loyola.edu.

• Movie reservations and showings  
• Computer lab reservation 
• VVR reservation (use of VVR for class, meetings, after-hours movie showings, cultural events, etc.)
• LLC equipment reservation (video camera, still camera, etc.)
• Event advertising assistance 
• Poster Request Form (use for non-event advertisements)
